Business Internet Banking E-Statement Agreement
Welcome to the Peshtigo National Bank’s online electronic bank statement delivery service (hereafter referred to as “e-statement”). This agreement covers the duties and responsibilities of both the customer (hereafter referred to as “you” or “your”) and Peshtigo National Bank (hereafter referred to as “we” or “the Bank”). Please read this agreement carefully.
To provide your business deposit account statement and required notices to you electronically only, the Peshtigo National Bank is required to obtain your consent of this agreement to the receipt of your statement. You also agree to receive electronically any and all required disclosures the Bank might send you with your checking account statement (for example, notices of changes in services or fees, Electronic Funds Transfer Disclosure, Funds Availability Disclosure, the annual Privacy Notice, etc.). Your consent to receiving documents in electronic format only applies to your deposit account statement. The same terms and conditions apply with respect to e-statements as for those delivered in printed form, and the deposit agreement and disclosures that you have previously entered into with or received from the bank remain in effect.
The Peshtigo National Bank will notify you by email when your electronic statement and required notices are available for viewing. If your email address to which we will be sending your notice should change, you must notify the bank promptly (either by phone, in writing, or by inserting the new email address on the PNB Online Banking system). If you do not update your new email address and your online statement is returned to us as undeliverable, the bank reserves the right to begin sending paper copies of your statement and required notices in the mail after a period of time. To view e-statements and required notices, you will need a computer with the appropriate Adobe Reader. You have the right to obtain duplicate paper copies of your current and prior deposit account statements while receiving electronic statements by contacting our BeB Department by phone at 1-888-550-4512 for a charge shown in our Deposit Account Fee Schedule, which may be amended by us from time to time. You have the right to withdraw your consent to have your account statement and required notices provided or made available only in electronic format by calling us at 1-888-550-4512 or in writing to the Peshtigo National Bank, Attn: BeB Dept., 250 French Street, Peshtigo, Wisconsin, 54157. Although you provide your consent to receive statements and all required records and notices electronically, Peshtigo National Bank retains the right to send these items by postal mail. Also, we may provide to you other notices and account related communications electronically not required by law.
You acknowledge and agree that the bank has no liability to you whatsoever for any loss, claim, or damages arising from or to, any email or other electronic communications that we have submitted to you or you to us. We also have no duty to investigate the validity or to verify any email or other electronic communications sent or received by the Peshtigo National Bank. Although we have no obligation to do so, we reserve the right to require authentication of emails or electronic communications. The decision to require authentication is at the sole discretion of the bank. This agreement, including the validity of any signatures or consents, any claim, or disputes arising hereunder shall be construed in accordance with and governed by the State of Wisconsin. To the extent the Bank may have breached any term of this consent and agreement, you agree that your sole remedy is to discontinue use of the e-statement service.
By selecting “I Accept” you consent to the receipt of Electronic Statements and Notice Disclosures electronically and you accept the terms and conditions of this Agreement.
By selecting “Cancel,” you do NOT consent to the receipt of Electronic Statements and Notice Disclosures electronically and you do NOT accept the terms and conditions of this Agreement.
